ACORD forms

ACORD forms are the standardised insurance application and certificate documents published by ACORD, a non-profit standards body, which US brokers use to submit risk information to carriers and MGAs in a consistent format.

The forms most often seen in a commercial submission are the ACORD 125 (Commercial Insurance Application, carrying the applicant, business and general information), the ACORD 127 (Business Auto Section, used for commercial auto and trucking), and the ACORD 140 (Property Section). Many programs add their own supplementals alongside them.

Although the forms are standardised, what arrives is often not. Forms are submitted as scans, as photographs, as fillable PDFs where the values live in form fields rather than in the visible page, and frequently with handwritten annotations or attachments that contradict the form itself. A system that reads ACORD documents reliably has to handle all of those cases, not just a clean digital PDF.

Recognising which ACORD form a page belongs to matters as much as reading it, because the same label means different things on different forms, and the fields that drive rating differ by line of business.
